It's important that you let us know who'd you'd like to receive any lump sum benefit from the Scheme if you die.

Although no one likes to think about what happens after they die, it's important that you let us know who you'd like to receive any lump sum benefit from the Scheme. Your Expression of Wishes form lets us know who you'd like to receive any Scheme benefits that are paid to your beneficiaries when you die, so it's really important that you keep it up to date.

While the Trustee will use their discretion when deciding who receives your death benefits, we always take your wishes into account and take note of the decisions you make in your form.

Tell us who should receive a lump sum benefit

Completing your form only takes a few minutes. Simply:

  1. Download a form from the Documents page
  2. Fill in the necessary sections
  3. Send it back to us at Abbott Laboratories Pensions Department, Abbott House, Vanwall Business Park, Maidenhead, SL6 4XE. Alternatively, if you are comfortable with personal information about yourself and your dependants being sent electronically, email it to us at ukpensions@abbott.com

Remember, you should update your preferences by completing a new Expression of Wishes form if your circumstances change, for example if you get married or have children.
